The Brainpan Medicare AIVI™ Benchmark
The Brainpan AIVI™ (AI Visibility Index) measures how often and how favorably Medicare plans and carriers are retrieved, cited, and recommended by AI answer surfaces. The first quarterly edition publishes October 1, 2026, scoring named Medicare brands on share of voice, per-mention quality (OVI), and citation behavior across seven AI answer surfaces — ChatGPT, Gemini, Claude, Perplexity, Google AI Overviews, Microsoft Copilot, and Bing Copilot.
Three patterns are visible before a single name publishes.
Three large national carriers hold just over half of all commercial-brand mentions in the study — concentration that mirrors what the Insurance AIVI™ found in Q2. But mention volume and answer quality diverge sharply: the brands named most often are not the brands ranked highest on quality once volume is taken out of the score. One smaller regional plan is discussed constantly and all but never cited as its own source. And one much smaller national challenger gets compared to the two largest incumbents more often than chance alone would predict — treated by the AI as a real rival, not an also-ran. Full entity extraction on those 14,405 responses produced 229,372 extracted mentions across 7,127 distinct entities — the complete Total Visibility Universe. Of that universe: 54,370 mentions (23.7%) are commercial-brand mentions, across 121 tracked Medicare brands (115 with recorded visibility). 5,073 mentions (2.2%) are government and advocacy organizations — split into 4,378 mentions across 766 government entities proper (CMS, Medicare.gov, the state SHIP/SHIIP/HICAP counseling network, the VA, state Departments of Insurance, and others) and 695 mentions across 20 advocacy/quasi-government organizations, which are named alongside government sources in the data but aren't government themselves.
