UnitedHealthcare's QVI™ rank is not an outlier within the top three reach leaders — Humana (36.62) and Aetna (34.77) also sit below several much smaller brands. But UnitedHealthcare's gap is the starkest in absolute terms: eleven times Medicare.org's mention volume, 13.14 points behind it on QVI™.

2. What we measured, and what we did not
This section is deliberately placed before the analysis rather than after it.
- Public model outputs only. Every figure comes from scored commercial-brand mentions in responses generated by ChatGPT, Gemini, Claude, Perplexity, Google AI Overviews, Microsoft Copilot, and Bing Copilot. No advertising data, no analytics access, no internal reporting from UnitedHealthcare or AARP.
- No client relationship. Brainpan.AI has no commercial relationship with UnitedHealthcare. The company was not contacted, did not participate, and did not review this page. It appears here because it is the commercial-share leader in the Q3 2026 study, not because of any dispute or engagement.
- Row-based, not response-based. QVI™ and position scores are computed per scored row; a response can name a brand more than once, and repeated rows can occupy several positions. UnitedHealthcare's QVI™, mean recommendation score, own-domain citation rate, top-3 rate, and mean position score are computed on the scored subset of its rows (marked † on the full leaderboard); its mention count and commercial share reflect the complete dataset.

The benchmark identifies 208 direct-comparison responses, where two or more carriers are weighed against each other in a single answer; 186 contain exactly two carrier brands. The higher Visibility Points total (the sum of position, recommendation, and citation scores) defines the scored winner within each pair.

UnitedHealthcare appears in the most head-to-head comparisons of any brand except Humana (78 against 114), which is itself a reach signal — it is named alongside a competitor often enough to be matched up repeatedly. But it converts those appearances into wins at the second-lowest rate in the published table. These outcomes do not necessarily represent explicit AI choices or beneficiary preferences, and opponent mix and sample size differ row to row; UnitedHealthcare's 78-appearance sample is large enough that the 35.9% figure is not a small-sample artifact the way Alignment Health's 16-appearance 93.8% might be.

Stop treating mention volume as the constraint
UnitedHealthcare already holds the largest mention share in the study by a wide margin. A program that opens with increase AI visibility and measures itself on mentions is optimizing a variable already performing at the top of the category. The first question is not how to be named more often. It is what happens in the sentence after the name, and how the answer resolves when UnitedHealthcare is matched directly against a competitor.
Close the authority gap relative to Humana and Aetna
UnitedHealthcare's 0.42 authority ratio trails both Humana (0.71) and Aetna (0.78), the two carriers closest to it on reach. The gap is specifically in owned-domain citation, not overall quality — uhc.com supplies 1.74% of captured citations against an 18.35% mention share. Closing even half that gap would move UnitedHealthcare's authority ratio closer to its nearest reach competitors without any change in mention volume.
Investigate the head-to-head loss pattern
A 35.9% win rate across 78 comparisons, second-to-last among the ten brands reported, is a large enough sample to warrant inspecting the underlying responses directly — which competitors UnitedHealthcare is most often matched against, and whether losses cluster around specific query types or Position, Recommendation, or Citation Score components.
